BILLING AND COLLECTIONS
The health center has several ways for patients to afford their medical and dental care. Listed below are the types of assistance that are provided.
Payment Methods
The health center will honor Debit, MasterCard, VISA, American Express and Discover cards for payment of services at the time of visit. Personal checks, money orders and cash are also accepted.
Sliding Fee Schedule
The health center offers a Sliding Fee Discount Program for individuals and families who do not have insurance or whose income is below 200% of the Federal Poverty level. A patient must provide proof of their income (W-2's, Individual Tax Return 1040, copy of entitlement checks, last 2 paystubs) at time of visit in order to be eligible for the program. The amount the health centers fees will be discounted will be based on the patients family size and income. The Federal Poverty Level Guidelines in conjunction with the Sliding Fee Schedule determines the patients discount. Patients will not be turned away based on their inability to pay. The patients ability to pay is determined by your annual income and family size according to the most recent U.S. Department of Health & Human Services Federal Poverty Guidelines.
Entitlements
The health center assists all patients in applying for third party assistance such as: Medicaid, Husky A and B. Please contact our Patient Support Deparment for information (860) 528-1359.
Payment Arrangements
The health center will offer you the option to establish payment arrangements to pay any outstanding balance, in a reasonable time period. Patients will not be turned away based on their inability to pay. The patients ability to pay is determined by your annual income and family size according to the most recent U.S. Department of Health & Human Services Federal Poverty Guidelines.
