First Choice Health Centers is a “place for the people”. Founded and built by neighborhood health activists in 1996, the Center is one of the newest in the tradition of Connecticut‘s community health centers. In attractive and modern facilities central to the neighborhood it serves, the Center provides a warm welcome and exceptional health care services.
in 2016 the Center celebrates our 20th anniversary with the addition of our co-located site with CHR in Manchester, a new site at 94 Union Street in Vernon which adds podiatry and behavioral health to Vernon and the conversion of 3 Prospect Street from medical to dental, a new service for the Vernon community as well. The Center has also purchased 809 Main Street in East Hartford to serve as our new administrative facility but also to house our many new specialty and primary care services to include internal medicine, geriatrics, behavioral health, optometry, podiatry and infectious disease. The Center will celebrate our anniversary on November 19, 2016 with a gala event to be held at the Rentschler Field banquet facilities in East Hartford. Congressman John B. First Choice provides comprehensive primary care from newborn through adult and elderly services. The Center offers family practice, internal medicine, pediatrics, dentistry, nutrition, sonography, geriatrics and in 2011 added podiatry followed by optometry in 2012. The Center added behavioral health services to our mix in 2014 and substance abuse services in 2016. Staffing includes licensed medical and dental providers including physicians, nurse practitioners, physician assistants, licensed clinical social workers, nutritionists, dentists, and dental hygienists. The Center also provides school based health services in 57 schools in East Hartford, Manchester, Vernon, most CREC schools and some CRT locations, including three schools that host full-time School Based Health Centers.
First Choice provides health care to 19,000 patients who made 85,000 visits in 2015 – a patient population that is 44% White, 26% Black or African-American, 34% Hispanic or Latino,6% Asian and 4% other.
